Malaysian construction industry had recorded the highest number of death compared to other industries. Few types of accidents cases in construction industry were reported to Department of Occupational Safety and Health (DOSH). For both fatalities and non-fatalities cases reported, accidents due to falls of person had the highest value for the years of recorded accidents statistic. Therefore, it is a major concern in this study to identify the root causation on the critical condition of fatal accidents due to falls of persons in Malaysian construction industry. 330 cases of fatal accidents in Malaysian construction industry, obtained from DOSH, were revised and 75 cases due to falls of persons were extracted from the database. The Loughborough Construction Accident Causation (ConAC) model was utilized in this study to analyze the accident causation. The findings show that the most frequent falls of persons-related accident types were falling from height. The most prevalent immediate accident circumstances causes were the local hazards. The two most commonly identified shaping factors were the physical site constraints and the lack of supervision. The main originating influence on the accidents was an inadequate risk management system. Accident pathways network was developed and the possible control measures were proposed respectively. This study is expected to be a stepping stone for the Malaysian construction industry to identify the related root causes in order to manage the prevention and control measures of commonly occurring accident types.
